3.5% Colloidal Infusion Solution For Plasma Substitution.

PRODUCT AND PRESENTATION:

HAEMACCEL 500ml: Pack of 1x500ml Polyethylene infusion bottle.

COMPOSITION:
1000 ml of Haemaccel contains: Degraded bovine gelatin polypeptides cross-linked via:
• urea bridges, 35g (equivalent of 6.3g of nitrogen)
• Sodium ions 145m.mol (3.33g),
• Potassium ions 5.1m.mol (0.20g),
• Calcium ions 6.25m.mol (0.25g),
• Chloride ions 145m.mol (5.14g). ions 145m.mol (5.14g).
Traces of phosphate ions and sulphate ions, plus anionic polypeptides up to the isoionic point.


Haemaccel Indications:

Haemaccel is a volume replacement used to correct or avert circulatory insufficiency due to plasma/blood volume deficiency, absolute (e.g. resulting from bleeding) or relative (e.g. resulting from a shift in blood volume between the circulatory compartments). Haemaccel is applied in the following areas: Hypovolemic shock, Loss of blood and plasma (e.g. due to trauma, burns, preoperative autologous blood or plasma donation), for filling the heart-lung machine. In addition Haemaccel can be used as a vehicle for various drugs.

POSOLOGY AND METHOD OF ADMINISTRATION:

Haemaccel is a ready-for-use solution to be infused by intravenous route. The speed and duration of the infusion depends on the need of the patient. The infusion speed is to be adjusted in accordance with the monitored blood pressure values. The drip rate can be calculated using the following formula: e.g. 500ml to be infused in 1 hour: 500/4x1(hr) =125 drops per minute.

In an emergency, Haemaccel may be administered rapidly (e.g. 500 ml over 5 to 15 minutes). For technical reasons, there is still some residual air  in the container. Thus, pressure infusions with the plastic infusion bottle must be carried out under carefully controlled conditions only, as the risk of an air embolism cannot be excluded. Reactions caused by histamine release can be avoided by the prophylactic use of H1 and H2 receptor antagonists (e.g dimetindene i.v. and cimetidine i.v.).

For physiological reasons Haemaccel should not be infused in the cold state, like all infusion solutions. Infuse clear solution only!

SPECIAL WARNINGS AND SPECIAL PRECAUTIONS FOR USE:

If incompatibility reactions occur, infusion must be stopped immediately. It is recommended that the guidelines of modern shock therapy be followed in severe reactions. At the latest when the hematocrit falls below 25%, consideration must generally be given to the administration of red cell concentrate or coagulation factors. The infusion of Haemaccel may result in a temporary increase in blood sedimentation rate.

INTERACTIONS WITH OTHER AGENTS:

In case of patients receiving cardiac glycosides, the synergistic effect of the calcium in Haemaccel should be taken into account.

PREGNANCY AND LACTATION:

There is no reason why pregnant women or nursing mothers should not use preparation preparations. Generally, however, particular care should be exercised when fluid or volume replacements are administered during or immediately after pregnancy.

UNDESIRABLE EFFECTS:

During or after the infusion of plasma substitutes, transient skin reactions (urticaria, weals), hypotension, tachycardia, bradycardia, nausea/vomiting, dyspnea, increases in temperatur. and/or chills may occasionally occur. Rare cases of severe hypersensitivity reactions sometimes progressing to shock have been observed.

OVERDOSE:

If large amounts of Haemaccel are given, the circulatory parameters should be monitored closely.

Haemaccel Dosage:

As prescribed by the Physician. Keep out of the reach of children. For intravenous administration. Sterile and pyrogen-free. Use only the contents of unopened bottles, the solution must be clear. Does not gel even if stored in a refrigerator.

CONTRA-INDICATIONS:

Known hypersensitivity to constituents of the preparation. Existing anaphylactoid reactions. Haemaccel is indicated only to a limited extent. If the physician thinks that the infusion is necessary, special precautions should be taken. All position of intravascular hypervolemia and their consequences (e.g. increased stroke volume, elevated blood pressure), or an increase in interstitial fluid volume or hemodilution could represent a special risk for the patient. Examples of such conditions are: congestive heart failure, hypertension, esophageal varices, pulmonary edema, hemorrhagic diathesis, renal and post-renal anuria. In all patients at increased risk of histamine release (e.g. Allergic people and patients with a history of reactions to histamine; as well as patients who have taken histamine-releasing medications within the previous 7 days).

In the following cases, Haemaccel can only be used after taking appropriate precautions.

STORAGE:

Store at room temperature. The solution must not be used after the date of expiry given on the pack and container. Once an infusion bottle has been opened, any unused contents are to be discarded.
